As a parent/guardian of a team player of AZ643 Baseball, by subscribing to monthly and tournament dues, I understand and have read the below:

o I understand if a player chooses to no longer participate on an AZ643 Baseball team, 30-day written notice will be needed to shut down dues.

o I understand that if I do not give the required written notice, I will be subject to one more payment of dues the following month. For example, if notice is given on October 2nd, you will responsible for dues the following month, November, before they are shut down.

o I understand that if my child is dismissed or voluntarily leaves, parents are not entitled to a refund and all monies are forfeited that have already been paid.

o I understand that if/when my player plays out state or out of the Phoenix metropolitan area, travel/lodging/meal expenses are my sole responsibility.
